What Is a Facelift (Rhytidectomy)?
A facelift, known medically as rhytidectomy, is a surgical procedure that lifts and tightens the deeper structures of the face to reverse visible signs of ageing. Unlike topical creams or injectable treatments that address only the surface, a facelift repositions the underlying muscle and fascia layer, removes surplus skin and restores a naturally youthful facial contour.
As we age, the skin loses collagen and elastin, fat pads descend, and the supporting connective tissue known as the SMAS (Superficial Musculo-Aponeurotic System) weakens. SMAS Facelift
The SMAS facelift is the gold-standard technique worldwide and the most commonly performed facelift at MedProper. It addresses the fundamental cause of facial ageing: the descent of the SMAS layer that lies beneath the skin.
An incision runs from the temple, follows the natural crease in front of the ear, curves beneath the earlobe and extends into the hairline behind the ear. The surgeon then elevates a skin flap, exposes the SMAS, and either plicates (folds and sutures) or elevates and repositions it. Excess skin is trimmed and closed without tension, which prevents the unnatural "pulled" appearance associated with older skin-only facelift methods.

Deep Plane Facelift
The deep plane facelift goes one layer deeper than the standard SMAS technique. This allows the mid-face tissues to be repositioned vertically rather than simply pulled laterally.

Mid-Face Lift

How Is a Facelift Performed? Step by Step

Step 1 — Anaesthesia
General anaesthesia is administered by a board-certified anaesthetist. You will be completely asleep and pain-free throughout the procedure. A mini facelift may be performed under local anaesthesia with sedation if the surgeon and patient agree this is appropriate.
Step 2 — Incision placement
The surgeon marks the incision lines, which follow the natural contours of the face: beginning in the temple hairline, continuing down the front of the ear (within the natural crease), looping beneath the earlobe and extending behind the ear into the posterior hairline. Step 3 — Skin flap elevation

Infection — rare with proper antibiotic prophylaxis and sterile technique, but possible with any surgery. Treated with antibiotics and wound care.

Visible or widened scars — most facelift scars heal beautifully, but individual healing varies. Scar revision can be performed if needed after 12 months.
Rare complications (less than 1%):
Nerve injury — temporary weakness of a facial nerve branch can occur, most commonly affecting the marginal mandibular nerve (lip movement). Permanent nerve injury is extremely rare (less than 0.5% in published studies) and most temporary weakness resolves within 3 to 6 months.
Skin flap necrosis — loss of blood supply to a small area of the skin flap. This is almost exclusively seen in active smokers and is a primary reason we require smoking cessation.

What is the difference between a SMAS facelift and a deep plane facelift?
A SMAS facelift lifts the SMAS layer separately from the skin, tightening it with sutures. A deep plane facelift releases the retaining ligaments and lifts the SMAS, fat and skin together as a single unit. The deep plane approach provides a more significant lift of the mid-face and nasolabial folds and typically produces longer-lasting, more natural results.
Will I have visible scars after a facelift?
Facelift incisions are placed within the hairline and along the natural creases in front of and behind the ear. Once fully healed (6 to 12 months), the scars mature into fine white lines that are very difficult to detect, even at close range. Your hairstyle will easily conceal any scarring.

When can I fly after a facelift?

When can I go back to work after a facelift?
Most patients return to desk-based work 2 to 3 weeks after surgery. If your job involves physical activity or face-to-face client interactions, you may prefer to take 3 to 4 weeks off. Residual swelling at the 2-week mark is subtle and easily concealed with light makeup.
When can I exercise after a facelift?
Light walking is encouraged from day 4 onwards. You can resume moderate exercise (gym, yoga, cycling) at 4 to 6 weeks. Contact sports and heavy weight lifting should wait until 8 weeks post-surgery.
Do I need to stop smoking before a facelift?
Absolutely. Smoking constricts blood vessels, dramatically reducing blood flow to the skin flap. This increases the risk of wound healing complications, skin necrosis and poor scarring. We require all facelift patients to stop smoking at least 4 weeks before and 4 weeks after surgery. This includes vaping and nicotine patches.
What type of anaesthesia is used for a facelift?
A full SMAS or deep plane facelift is performed under general anaesthesia. A mini facelift may be performed under local anaesthesia with intravenous sedation (twilight sedation). Your surgeon and anaesthetist will recommend the safest option based on the scope of your procedure and your medical history.
How do I choose between a mini facelift and a full facelift?
The choice depends on the degree of ageing. If you have early jowling and mild skin laxity but no significant neck issues, a mini facelift may be sufficient. If you have pronounced jowls, deep nasolabial folds, mid-face descent or neck laxity, a full SMAS or deep plane facelift will produce a far superior result. Your surgeon will guide you during the consultation.
Will a facelift change my facial expressions?
No. Modern facelift techniques reposition deep tissue structures without affecting the muscles of facial expression. You will smile, frown and laugh exactly as you do now. The only difference is a more youthful contour. The "frozen" look sometimes associated with facelifts is a result of outdated skin-only techniques that applied excessive tension — a method no longer used at MedProper.
Can men have facelifts?
Yes. Male facelifts account for approximately 10 to 15 percent of our facelift patients. The technique is slightly modified to account for thicker male skin, a different hairline pattern and the presence of facial hair. Incisions are placed to avoid disrupting sideburns and beard growth. Recovery is similar, though men tend to experience slightly more bruising due to greater skin vascularity.
