The surgeon or extraction team creates tiny circular incisions around each follicular unit, loosens the graft from surrounding tissue, and extracts it intact.
Extracted grafts are stored in a chilled holding solution to preserve viability while the recipient channels are created. Sapphire FUE is an advanced variant that uses blades tipped with sapphire crystal (aluminium oxide, the second-hardest natural mineral after diamond) instead of steel to create the recipient channels. The sapphire blade creates a V-shaped micro-incision rather than the U-shaped incision of a steel slit.
Why sapphire matters:
The extraction phase is identical to standard FUE — the difference is entirely in the channel-creation step. This means Sapphire FUE carries no additional risk compared to standard FUE.

A Mega Session is a high-volume FUE procedure performed over two consecutive days to transplant 5,000 to 6,000 grafts. This is designed for patients with advanced hair loss (Norwood 5-6) who need maximum coverage in a single treatment cycle.
Day 1: Extraction of all grafts + implantation of approximately half (typically the frontal zone and mid-scalp).
Day 2: Implantation of remaining grafts into the crown and any areas requiring additional density.
Splitting the procedure over two days reduces fatigue for both the surgical team and the patient, maintains graft viability (grafts are extracted and implanted on the same day they are harvested), and allows the surgeon to assess Day 1 results before finalising Day 2 placement.

Step 1 — Hairline Design. Your surgeon draws the proposed new hairline while you sit upright, using landmarks (forehead creases, facial proportions, age-appropriate recession) to create a natural, balanced result. You approve the design before proceeding.
Step 2 — Donor Area Preparation. The donor area (back and sides of the head) is trimmed to 1-2mm to allow precise punch placement. For no-shave FUE (available with DHI), only the donor area is shaved.
Step 3 — Local Anaesthesia. Anaesthetic is administered to the donor and recipient areas. MedProper uses needle-free comfort anaesthesia (a pressurised injector) for the initial numbing, followed by supplemental local anaesthetic as needed. The procedure is completely painless once anaesthesia takes effect.
Step 4 — Graft Extraction. The surgeon or extraction team uses the micro-punch to extract individual follicular units. Each punch creates a circular incision less than 1mm in diameter. Grafts are carefully removed and placed in a chilled holding solution (typically Hypothermosol or chilled saline) to preserve viability.
Step 5 — Channel Creation. The surgeon creates recipient channels (micro-incisions) in the bald or thinning area. This is the most critical phase — the angle, depth, direction, and density of these channels determines the naturalness of the final result. Step 6 — Graft Implantation. Grafts are placed into the channels using fine-tipped forceps (or Choi implanters in the case of DHI). Single-hair grafts are placed at the very front of the hairline for a natural feathered edge. Multi-hair grafts (2-4 hairs) are placed behind the hairline for density.
Step 7 — Post-Operative Care. The donor area is left open to heal (no bandaging required for FUE). The recipient area is gently bandaged with a protective headband. You receive aftercare instructions, medication, and a follow-up appointment for the next day.
