What Is Gynecomastia?

Gynecomastia is the enlargement of male breast tissue caused by an imbalance between oestrogen and testosterone. It affects an estimated 50 to 65 percent of men worldwide and can develop at any stage of life -- during puberty, in middle age, or later. The condition ranges from a small amount of firm tissue behind the nipple to significant breast-like fullness that is visible through clothing.

The causes are varied. Hormonal shifts during puberty or ageing are the most common trigger, but medications (anabolic steroids, anti-androgens, certain antidepressants), recreational substances, liver or kidney conditions, and simple genetics all play a role. In many cases no specific cause is ever identified, and the tissue does not resolve on its own.

While gynecomastia carries no serious medical risk, the psychological burden is well documented. Men describe avoiding the gym, the beach, fitted shirts, and intimate situations. The self-consciousness can persist for years or decades before they seek help. If you recognise yourself in any of that, you are not alone -- and surgery offers a permanent, reliable solution.

At MedProper Istanbul, gynecomastia surgery is performed by board-certified plastic surgeons who are members of ISAPS (International Society of Aesthetic Plastic Surgery) and TSAPS (Turkish Society of Aesthetic Plastic Surgery). Our facility holds JCI accreditation, the gold standard in international hospital safety, and our all-inclusive package is designed specifically for male patients travelling from the UK, USA, and Europe.

Gynecomastia Grading System

Surgeons classify gynecomastia into four grades. Your grade determines which surgical technique delivers the best result.

Grade I -- Minor Enlargement, No Skin Excess

A small button of firm tissue sits directly behind the nipple. The chest appears normal in most clothing but the tissue is palpable and sometimes tender. Liposuction alone is usually sufficient.

Grade II -- Moderate Enlargement, No Skin Excess

The breast tissue extends beyond the areola boundary. A noticeable fullness is visible in fitted shirts and when the arms are raised. Liposuction combined with gland excision through a periareolar incision is the standard approach.

Grade III -- Moderate Enlargement with Skin Excess

Larger breast volume is present alongside mild to moderate skin laxity. The areola may have stretched. Combined liposuction and excision are required, and a periareolar skin tightening technique is often added to redrape the skin.

Grade IV -- Severe Enlargement with Significant Skin Excess

The chest closely resembles a female breast with substantial tissue volume and hanging skin. This grade requires excision of both gland and skin, often with repositioning of the nipple-areola complex. The surgical plan is more extensive but the transformation is dramatic.

During your free online consultation, your MedProper surgeon will review photos, determine your grade, and recommend the most appropriate technique before you travel.

Types of Gynecomastia Surgery

There is no single operation for gynecomastia. The right technique depends on your grade, the ratio of fatty tissue to glandular tissue, skin quality, and your aesthetic goals.

Liposuction Only

Best for: Grade I and some Grade II cases where the enlargement is primarily fatty tissue with minimal glandular component.

The surgeon creates one or two tiny incisions (3 to 4 mm) at the edge of the areola or in the armpit crease. A thin cannula connected to a vacuum device breaks up and removes the excess fat. At MedProper, we use VASER (ultrasound-assisted) liposuction for chest work because the ultrasonic energy selectively targets fat cells while preserving blood vessels, nerves, and connective tissue. This means less bruising, smoother contours, and better skin retraction.

The procedure takes 45 to 60 minutes per side. Scars are virtually invisible once healed.

MedProper price: EUR 2,160 all-inclusive.

Gland Excision

Best for: Cases where firm, rubbery glandular tissue sits directly behind the nipple and cannot be removed with liposuction alone.

Through a small semicircular incision along the lower border of the areola (periareolar approach), the surgeon directly visualises and removes the dense gland. The incision follows the natural colour transition between areola and chest skin, making the scar extremely discreet once mature.

Excision is essential when hard glandular tissue is the primary component. Attempting liposuction alone on dense gland tissue leads to incomplete correction and an unnatural residual lump beneath the nipple.

MedProper price: EUR 2,810 all-inclusive.

Combined Liposuction + Gland Excision

Best for: Grade II, III, and most Grade IV cases where both fatty and glandular tissue contribute to the enlargement.

This is the most commonly performed gynecomastia operation worldwide. The surgeon begins with VASER liposuction to remove peripheral fat and define the chest contour, then switches to direct excision through the periareolar incision to remove the central gland. The combination produces the most complete, natural-looking result.

For Grade III and IV cases with skin excess, the surgeon may add a periareolar purse-string closure to tighten the skin envelope, or in severe cases, remove a crescent or full donut of skin around the areola.

MedProper price: EUR 3,170 all-inclusive.

Skin Reduction Techniques (Grade III-IV)

When significant skin laxity is present, the surgeon has several options depending on severity:

  • Periareolar (donut) lift: Excess skin is removed in a ring around the areola. Suitable for mild to moderate laxity.
  • Extended excision: For severe cases, an incision extends laterally from the areola. This is less common but necessary when large amounts of skin must be removed.
  • Nipple repositioning: In Grade IV, the nipple-areola complex may need to be moved to a higher, more masculine position on the chest wall.

Your surgeon will discuss these options during consultation and show you before-and-after examples of patients with a similar grade.

Why Choose Turkey for Gynecomastia Surgery?

Significant Cost Savings

Gynecomastia surgery in the UK typically costs GBP 4,000 to GBP 6,000 for the surgeon fee alone, with anaesthesia, hospital, and compression garments charged separately. In the USA, expect USD 5,000 to USD 8,000. At MedProper Istanbul, an all-inclusive package starts at EUR 2,160 -- saving you 50 to 70 percent even after flights and accommodation are factored in.

Surgeon Expertise in Male Chest Contouring

Turkish plastic surgeons perform a very high volume of body contouring procedures -- including gynecomastia -- year-round. This volume translates into refined technique, especially for VASER-assisted cases and combined lipo-excision approaches. MedProper surgeons are board-certified, ISAPS-affiliated, and specialise in male aesthetics.

JCI-Accredited Hospital Standards

Your surgery takes place in a JCI-accredited hospital in Istanbul. JCI (Joint Commission International) is the same accreditation body that certifies leading hospitals in the USA, UK, and Europe. Infection control, anaesthesia protocols, and post-operative monitoring meet the same standards you would expect at home -- often in newer, purpose-built facilities.

Privacy and Discretion

Many men delay gynecomastia surgery because they feel embarrassed discussing it with their local GP or surgeon. Travelling to Istanbul removes that barrier entirely. You consult privately online before travel, undergo surgery in a city where nobody knows you, recover in a comfortable hotel, and return home with the results already taking shape. The entire process is discreet from start to finish.

All-Inclusive Package -- No Hidden Costs

MedProper's gynecomastia package covers the surgical fee, anaesthesia, one night in hospital, six nights in a four-star hotel, all ground transfers (airport, hotel, hospital), a compression vest, post-operative medications, and follow-up appointments in Istanbul. The only additional cost is your flight.

Your 7-Day Journey: Gynecomastia Surgery in Istanbul

Day 1 -- Arrival and Consultation

Your private transfer meets you at Istanbul Airport and takes you directly to your hotel. After settling in, you attend your in-person consultation at the clinic. Your surgeon examines your chest, confirms the surgical plan discussed during your online consultation, takes clinical photographs, and answers any remaining questions. Blood tests and an ECG are performed at the hospital.

Day 2 -- Surgery Day

Your transfer collects you from the hotel early in the morning. At the hospital you meet your anaesthesiologist and surgical team. The operation takes one to two hours depending on technique. You wake in a private recovery room wearing your compression vest. You stay overnight in the hospital for monitoring.

Day 3 -- Hospital Discharge and First Check

Your surgeon visits you in the morning, checks the surgical site, and reviews your recovery instructions. You are discharged to your hotel. Light walking is encouraged from day one. You may notice swelling and bruising -- this is normal and expected.

Days 4-5 -- Rest and Light Exploration

Most men feel well enough to walk around the hotel, eat at nearby restaurants, and take short outings in Istanbul. Keep your compression vest on at all times except when showering (you can shower from day three with waterproof dressings). Avoid lifting anything heavier than a few kilograms.

Day 6 -- Second Follow-Up

You return to the clinic for a wound check, dressing change, and review of your recovery progress. Your surgeon confirms you are safe to fly and provides a written medical report, aftercare instructions, and emergency contact details for use after you return home.

Day 7 -- Departure

Your private transfer takes you to Istanbul Airport. Most men fly home comfortably in their compression vest. The vest is worn under clothing and is not visible.

Recovery Timeline: When Can You Return to the Gym?

TimeframeWhat to Expect
Days 1-3Moderate soreness, swelling, and bruising. Prescription painkillers manage discomfort. Light walking only.
Week 1Bruising begins to fade. Swelling peaks around days 3-5, then starts to improve. You can return to desk work.
Weeks 2-3Significant reduction in swelling. Most bruising resolved. You may resume light lower-body exercise (walking, stationary cycling).
Week 4Compression vest wear reduces to nighttime only (surgeon's discretion). Light upper-body exercise may resume -- no heavy chest work yet.
Weeks 6-8Most swelling resolved. You can gradually return to full gym workouts including chest exercises. Scars are pink and will continue to fade.
Month 3Near-final result visible. Residual firmness under the nipple is normal and continues to soften.
Month 6-12Final result. Scars mature to thin white lines. Chest contour is stable and permanent provided you maintain a stable weight.

Important for gym-goers: The chest muscles are not cut during gynecomastia surgery. Your strength and muscle mass are preserved. The tissue removed sits on top of the pectoral muscle. Once healed, you can train your chest as intensely as before -- and you will finally see the definition you have been working for.

Can I Combine Gynecomastia with Other Procedures?

Yes. Many men choose to address multiple concerns in a single trip to Istanbul. The most popular combinations include:

Gynecomastia + Abdominal Liposuction

The most common combination. While the surgeon is already performing VASER liposuction on the chest, extending the treatment to the abdomen and flanks adds relatively little operating time. The result is a comprehensively reshaped upper body. This combination is especially effective for men who carry stubborn belly fat alongside chest fullness.

Gynecomastia + Six-Pack Etching (HD Lipo)

For men with good underlying abdominal muscle tone who want a sculpted, athletic look. The surgeon combines chest reduction with high-definition VASER liposuction of the abdomen, creating visible abdominal definition. This is a premium technique suited to patients who are already relatively lean.

Gynecomastia + Rhinoplasty

A less obvious but increasingly popular combination for men who want to address two long-standing insecurities in one trip. Since rhinoplasty involves a completely different surgical field (the face), there is no conflict with chest surgery. Both procedures are performed under the same general anaesthetic, and recovery timelines overlap comfortably.

Discuss combination options during your free online consultation. Your surgeon will advise on what can safely be performed in a single session based on your health, BMI, and the complexity of each procedure.

Gynecomastia Surgery Results: What to Expect

Immediate Post-Surgery

Your chest will be noticeably flatter even on the operating table, but significant swelling masks the final shape for several weeks. The compression vest controls swelling and supports the new contour. Do not judge your result in the first two weeks.

Weeks 2-6

Swelling reduces progressively. By week four, most men see a dramatic improvement and feel confident removing their shirt. The chest takes on a firmer, more masculine contour. Scars around the areola are pink but flat and easily concealed.

Months 3-6

The near-final result emerges. Any residual firmness beneath the nipple (common after gland excision) continues to soften. Scars fade from pink to pale. The chest looks and feels natural.

Long-Term (12 Months and Beyond)

The result is permanent. Removed glandular tissue does not grow back. Removed fat cells do not regenerate. However, significant weight gain, anabolic steroid use, or medications that affect hormones can cause remaining tissue to enlarge. Maintaining a stable weight and avoiding known triggers ensures a lasting outcome.

Scarring

  • Liposuction only: Scars are 3 to 4 mm dots at the edge of the areola or in the armpit. They fade to near-invisible within months.
  • Gland excision: A semicircular scar along the lower areola border. The colour match between areola and scar makes it very discreet.
  • Skin excision: A full periareolar scar (around the entire areola) is more visible initially but matures to a fine line within 12 months.
