A closed sinus lift (crestal approach) is a minimally invasive technique that elevates the maxillary sinus floor through the implant socket itself, without creating a separate window in the jawbone. It is used when a small amount of additional bone height is needed beneath the sinus for implant placement. The procedure is typically combined with simultaneous implant insertion, making it faster and less invasive than the open technique.

1A CBCT scan confirms that the available bone height is close to (but not quite) sufficient for implant placement (typically 5-8 mm available).
2Under local anaesthesia, the implant osteotomy (drill hole) is prepared to within 1 mm of the sinus floor.
3A specialised osteotome or hydraulic device gently taps upward, lifting the sinus membrane through the prepared socket.
4Bone graft material is introduced through the socket to fill the space created beneath the elevated membrane.
5The dental implant is placed into the same osteotomy with good primary stability, and the site is sutured.
