An open sinus lift (lateral window technique) is a surgical procedure that elevates the sinus membrane from the inside of the upper jaw and places bone graft material beneath it, creating sufficient bone height for dental implant placement in the posterior upper jaw. When natural bone below the sinus is too thin for implants, this well-established technique predictably regenerates the needed volume. - 1A CBCT scan measures the available bone height below the maxillary sinus and plans the graft volume required.
- 2Under local anaesthesia, a window is carefully created in the lateral wall of the upper jawbone.

- 4Bone graft material is packed into the space between the jaw and the elevated membrane.

A closed sinus lift is a simpler procedure done through the implant socket, suitable when only 2-3 mm of additional bone is needed. An open sinus lift uses a lateral window to access the sinus, allowing much larger volume gains (5-12 mm) for cases with severe bone deficiency.
The procedure is performed under local anaesthesia and is painless during surgery. Post-operative discomfort is moderate, similar to a wisdom tooth extraction, and is managed well with prescribed medication.
If there is at least 4-5 mm of existing bone for initial implant stability, simultaneous sinus lift and implant placement may be possible. Otherwise, the graft must heal first and implants are placed in a second procedure.
The main risk is perforation of the sinus membrane (occurs in about 10-25% of cases), which is usually repaired during surgery. Infection and graft failure are rare. Following post-operative instructions (no nose blowing, no smoking) significantly reduces risks.
Typically two trips: the first for the sinus lift (3-5 days), the second for implant placement 6-9 months later (2-3 days). If implants are placed simultaneously, only one additional trip for the final crowns is needed.
